OverviewFLSA: Non-Exempt This is a certified nursing assistant position at Lakeview Health Center & Assisted Living of La Crosse County. The Director of Nursing and/or designee appoints employees. Employees work under the supervision of a Registered Nurse, a Licensed Practical Nurse or a Nurse Technician.
Location: Lakeview Health Center & Assisted Living is a state-of-the-art facility in West Salem, WI.
Hours of work: 10 PM to 6:30 AM, every other weekend. Night shift weekends are Friday & Saturday. Additional hours are available! Staff also work a holiday rotation.
Wage depends on experience!
- Up to $22.89/hr and $24.89/hr on weekends with differential.

Essential Job Functions
Assists residents in all areas of personal hygiene, activities of daily living and care of their room and personal belongings. Encourages the highest practicable level of independence.(1) Assists residents at mealtime including assisting with eating and supervision of residents. Follows guidelines on individual diet card before giving each resident their meal.(1) Attends in-service education as provided or mandated. Meets federally mandated 12-hour in-service requirement annually.(3) Provides supervision to maintain safety of the residents at all times.(2) Follows directions and performs cares as assigned by the nurse and reports progress with those assignments to the nurse. Documents care provided to residents as directed.(1) Reports changes in resident's condition immediately to nurse for appropriate direction and intervention.(3) Performs simple duties such as temperature, pulse, respiration, weights, and skin treatments such as lotions or creams to closed skin.(1) Participates in defining needs of residents to assist in development of an individualized care plan for each resident.(3) Gathers and documents appropriate data on residents such as specific behaviors and restorative programs.(3) Follows all care plan approaches including using mechanical lifts and those developed by all other disciplines and may include specialized psychiatric services.(2) Works safely and follows safety practices and standards of the facility; responsible for reporting or correcting any existing or potential safety or accident hazard.(2) Maintains residents' rights and confidentiality of resident information; immediately reports all suspected cases of resident abuse to the Supervisor or Administrator.(3) Employee is expected to report to work when scheduled, with minimum use of sick leave, to build up sick leave balance for income protection for unexpected illness.(3) In this public service position, employee is required to be courteous, cooperative and respectful at all times, with the public and clients. Also establishes and maintains a courteous, cooperative and respectful working relationship with other employees, supervisors and public officials.(3) Supports all aspect or person-centered care. Respects and acknowledges individuality and uniqueness of residents.(3)Physical demands: Uses near vision, stands, walks, moves around the nursing home 6.5 to 7 hours on an 8-hour shift. Uses hands for delivering personal care and treatments. Communicates with residents and other staff members using talking and hearing. Read and write English. Ability to kneel or squat within six inches of the floor, pushes and pulls linen carts and wheelchairs with residents weighing up to 250 pounds or more. Lifts 35 pounds and transfers up to 50 pounds. Uses nursing-related equipment such as: scales; mechanical patient lifts (those under 18 are unable to operate the mechanical lifts alone per regulation); wheelchairs; adaptive/restorative equipment; tubs; and transports oxygen.
RELATED JOB FUCTIONS
Uses a gait belt and proper body mechanics when involved in lifting, pushing or pulling activities.(1) Maintains a safe, clean environment free of hazards. After meals, assures that dining area floors, tables, chairs and wheelchairs, etc are cleaned of food and liquids.(2) Escorts individuals or groups to dining room and other facility activities. (2) Takes an active role in living area and team meetings.(3) C.N.A.'s who have completed an approved medication certification course will pass medication under the supervision of a Registered Nurse.(1) Safely prepares, administers and records medications ordered by physician.(1) Administers medications as specified in the established facility medication administration policy and procedures. Maintains awareness of actions of each medication, usual dosage and route, and any change in resident condition before administering medication.(3) CNA who is assigned restorative programs will carry out recommendations/care plans for restorative care as developed by the Occupational Therapist (PT), Physical Therapist (PT), MDS Nurse and/or Nurse Managers.(2) Promote residents' involvement in the Restorative Activity Program. Assist resident with ambulation for therapeutic purposes. Transfer residents. Use proper body mechanics and appropriate assistive devices such as a gait belt when involved in lifting, pushing and pulling activities during transfers and while ambulating residents.(2) This job description is not intended to encompass every job duty or responsibility but is only illustrative. Knowledge of the principles of resident care, good hygiene, infection control and simple housekeeping.Ability to understand and follow oral or written directions.
Ability to conscientiously observe facility rules, regulations and policies.
Training, Experience and Other Requirements
Must be Certified Nursing Assistant at time of hire and listed on the Wisconsin Nurse Aide Registry.
Must be at least 16 years of age.
Lakeview driving: Employee must possess a valid driver's license and be approved by the County, if driving facility vehicles in the transportation of residents to program events, medical or dental appointments, etc.
Must successfully pass a background check as required by the Wisconsin Caregiver law.
