Job Summary
Performs outbound calls to provider offices and health systems to request for medical documentation following episodes of care. Ensures required information is included on the documentation. Tracks and follows up work in systems. Escalates delays or issues to leadership.
Education & Experience
Required:
• High school diploma or G.E.D.
• 1+ years working in health care operations or medical practice
• Strong communication and customer service skills
• Proficient with MS Office and computer applications
Preferred:
• Medical documentation or records experience
• Medical terminology knowledge
Key Responsibilities
• Views queues, opens chase requests and reviews documentation for required demographic information.
• Identifies claims and providers that need outreach.
• Performs outbound calls and other forms of outreach to provider offices and health systems to collect medical records.
• Completes notes and pends cases.
• Prepares coversheet and medical documentation for VA. Uploads document package to be pushed to VA Portal.
• Reports completed medical documentation collection or chase for clinical audit.
• Ensures contract compliance for timelines regarding resolution of medical documentation review and updates.
• Communicates effectively with management and peers.
• Consistently meets performance metrics for position.
• Performs other duties as assigned.
• Regular and reliable attendance is required.

Team-Building / Team Player: Influence the actions and opinions of others in a positive direction and build group commitment.
Technical Skills: Thorough knowledge of policies and procedures, Managed Care concepts and medical terminology. Proficient with claim and coding tools such as Find-a-Code, Current Procedural Terminology, Health Care Financing Administration Common Procedure Coding System, and American Dental coding. Ability to meet or exceed production standards in compliance with contract.
Working Conditions
Working Conditions:
• Must be available to cover any work shift
• Works within an office environment with minimal travel required.
• Extensive computer work with prolonged sitting.
