Kern River Transitional Care Center is seeking a dedicated and compassionate Physical Therapist to join our rehab team on a Per Diem basis. If you're passionate about helping patients regain strength and independence while enjoying flexible hours, we'd love to meet you!
Responsibilities:

• Evaluate and assess patients to develop personalized treatment plans
• Provide high-quality, hands-on therapy in accordance with physician orders
• Accurately document treatments and patient progress
• Communicate effectively with the interdisciplinary team
• Educate patients and caregivers on at-home therapy and safety strategies

Qualifications:

• Active California PT license in good standing
• CPR certification
• Experience in skilled nursing or rehab preferred
• Excellent interpersonal and documentation skills
