Your Role
As a Physical Therapist, you'll play a vital role in helping our residents achieve their rehabilitation goals. You'll work closely with physicians, nursing staff, and Physical Therapy Assistants to deliver exceptional care and promote the highest possible quality of life.
Key Responsibilities:
- Assessment & Treatment Planning: Perform thorough patient evaluations and create individualized treatment plans in coordination with the referring physician.
- Patient Care: Deliver skilled therapy to improve mobility, manage pain, and restore functional independence.
- Supervision & Collaboration: Supervise Physical Therapy Assistants and work collaboratively with the interdisciplinary care team.
- Documentation & Compliance: Complete timely and accurate clinical documentation per state and federal standards.
- Discharge Planning: Educate patients, families, and caregivers on maintenance programs and safe discharge procedures.
- Equipment Recommendations: Recommend and arrange for necessary durable medical equipment (e.g., walkers, braces, adaptive devices).
- Staff Development: Participate in in-service training and mentorship opportunities to support team growth.

Qualifications
- Valid Physical Therapist License - State of California (required)
