Resident Care ManagerMountain View Post Acute - A Great Place to Work
Wage: $35-$60 per hour (depending on licensure)
RN: $44-$60 (DOE) & LPN: $35-$47 (DOE)

Key Responsibilities
Care Coordination
- Develop, update, and monitor individualized resident care plans.
- Ensure plans reflect the physical, emotional, and psychosocial needs of each resident.
- Complete routine assessments and report changes in condition to the appropriate providers.
Resident Care & Advocacy
- Monitor resident health and provide direct nursing care as needed.
- Promote dignity, safety, and resident rights at all times.
- Serve as the main point of contact for families regarding resident care.
Team Leadership
- Support and supervise CNAs and nursing staff.
- Provide coaching, feedback, and ongoing training.
- Assist with performance evaluations and corrective action processes.
Documentation & Compliance
- Accurately document assessments, interventions, and care in the medical record.
- Follow state, federal, and facility regulations, including infection control and safety protocols.
- Participate in state surveys, audits, and quality assurance programs.
Communication & Collaboration
- Coordinate care with physicians, therapists, and other healthcare professionals.
- Facilitate resident and family care conferences.
- Report changes, incidents, and concerns promptly to the Director of Nursing.
What You Bring
- Current, unencumbered LPN or RN license in Washington State (required).
- 2 years of skilled nursing, long-term care, or similar experience preferred.
- Strong understanding of long-term care regulations.
- Excellent communication, organization, and leadership skills.
- Ability to prioritize, multitask, and adapt to resident needs.
