RN: $44-$60 (DOE) & LPN: $35-$47 (DOE)

Key Responsibilities

Care Coordination

• Develop, update, and monitor individualized resident care plans.
• Ensure plans reflect the physical, emotional, and psychosocial needs of each resident.
• Complete routine assessments and report changes in condition to the appropriate providers.

• Monitor resident health and provide direct nursing care as needed.
• Promote dignity, safety, and resident rights at all times.
• Serve as the main point of contact for families regarding resident care.

• Support and supervise CNAs and nursing staff.
• Provide coaching, feedback, and ongoing training.
• Assist with performance evaluations and corrective action processes.

• Accurately document assessments, interventions, and care in the medical record.
• Follow state, federal, and facility regulations, including infection control and safety protocols.
• Participate in state surveys, audits, and quality assurance programs.

• Coordinate care with physicians, therapists, and other healthcare professionals.
• Facilitate resident and family care conferences.
• Report changes, incidents, and concerns promptly to the Director of Nursing.

What You Bring

• Current, unencumbered LPN or RN license in Washington State (required).
• 2 years of skilled nursing, long-term care, or similar experience preferred.
• Strong understanding of long-term care regulations.
• Excellent communication, organization, and leadership skills.
• Ability to prioritize, multitask, and adapt to resident needs.
