Dublin Post Acute Care is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our therapy team! We are a skilled nursing and rehabilitation center committed to providing high-quality care, improving patient outcomes, and supporting our residents in achieving their highest level of independence.
Position Summary
The SLP will assess, diagnose, and provide treatment for residents with speech, language, cognitive-communication, voice, and swallowing disorders. You will work collaboratively with an interdisciplinary team to develop individualized care plans and deliver exceptional therapy services.
Responsibilities
- Conduct evaluations and develop comprehensive treatment plans.
- Provide therapy to residents with communication and swallowing disorders.
- Document progress notes, assessments, and treatment outcomes accurately and timely.
- Participate in care plan meetings and collaborate with nursing, therapy, and interdisciplinary teams.
- Educate residents, families, and staff on communication and swallowing strategies.
- Ensure compliance with state, federal, and facility regulations.
- Master's degree in Speech-Language Pathology.
- Current Ohio SLP license in good standing.
- ASHA CCC-SLP preferred (will consider CFY).
- Experience in skilled nursing or long-term care is a plus but not required.
- Strong communication, organizational, and clinical reasoning skills.
