TheSocial Worker assists with the acclimation of admission for each resident as well as discharge planning upon leaving the facility. They provide direct resident and family support and acts as a resident advocate while working collaboratively with our nursing and management team to provide the best experience possible. • Must have a Bachelors degree in Human Services, Social Work, Psychology, or related field
• Social work experience preferred
• Must have a valid Virginia driver's license
• Experience in long-term care preferred
• Ability to communicate effectively with a variety of individuals
• Knowledge of social services practices and procedures
• Leadership skills
• Organizational skills
• Ability to make independent decisions
• Ability to communicate effectively both orally and in written form
