Sonoma Post Acute is seeking a Telehealth Physical Therapist to provide high-quality virtual care to our residents. This role is ideal for a licensed PT who values flexibility, strong teamwork, and delivering effective patient outcomes through telehealth.

Responsibilities

• Conduct virtual PT evaluations and follow-up treatments
• Develop individualized care plans
• Document accurately in the EMR
• Collaborate with onsite nursing and rehab teams
• Support continuity of care for post-acute residents

• Active California Physical Therapist license
• Experience in SNF, post-acute rehab, outpatient, or telehealth preferred
• Strong communication and documentation skills
• Comfortable working independently in a remote setting

• Starting rate: $75 per hour
